Tony Stark always thought that he could only live with the metal heart that he had to build in the cave when his own heart was giving out due to the bullet that was lodge in it. When he had to build one for Pepper to live - he knew that he had to find another way for the love of his life to live. Which he managed too do. He also learned that he could live a life without the metal heart beating in his own chest.
