This isn't a SI/OC but a SI as Itachi. SI/OC literally means Self Insert/Original Character, which is not the case here. Man he'll be blind as a bat real quick, plus the drain alone might kill him. Will he keep the Uchiha talent, considering that his brain is an Uchiha's? If not it really won't be him.

The fire scene was very easy to visualise. Her frenzied thoughts too.
As for wrong gender SIs, there's Dysphoria, and a few others-can't remember the names now, though.
Traumatised-enough-to-reincarnate-with-mangekyou-girl-as-m!Itachi. There's so much potential in this.

I love the style in which you decided to start with. There are many OC/SI stories and it's always interesting to see how authors decide to start them. I think you've introduced your character very skillfully. And being born with the most tragic of fancy eyes is a neat twist.
