Hi, I am brazilian and I am reading your story. Fomulário actually is a substantive, not a verb. It means forms, like paper forms. And Conexão means Connection. So, for this sentence actually means Form Connection it should be Forme Conexão.
I am really liking the story. Hope the following chapters are also nice.
